There's a bug with the sliding pillars. Sometimes it seems if you open the menu or something, one of the pillars will slide out of position. This means if you're past the pillars, it's impossible to go back to the entrance and you're stuck. Pressing Shift doesn't fix this. The dungeon really needs a 'Exit To World Map' option given how buggy it is.

The dungeon really needs a 'Exit To World Map' option given how buggy it is. Are you talking about the block push puzzle at the beginning of the dungeon? If you get past this part and reach the roof outside, you can push a small obstacle out of the way of a vine to create and shortcut back down. There's also a vine shortcut back to the entrance connecting with a door on one of the levels in the block push room. You can also use the sand slides on various levels of the block push room to get back down to the start. Those are some workarounds, but they don't excuse the bug. I've been over the code numerous times and couldn't figure out why the results can often turn erroneous. At this point I can't really do much but blame the engine. Your suggestion of an "Escape Dungeon" option is a good one, and I'd like to implement that if I get a chance. It might get a little complicated since various puzzles would have to be reset, but I'll see what I can do.
