Okay, I get this one thing during battle pretty often and it doesn't seem to be at any single place in the game although i haven't noticed it in the start of the game. I mostly use Glen, Meredith, Justin and Vixen and whenever I'm in a battle, Meredith always seems to be skipped in the first round(I can choose what to do, but the choice doesn't register), and then they just go on to the next character as if i choose defend on that one.
It only happens in the first round in each fight, and I don't know if it's only the second character (in the battle order/list) that get this Failed-Command - Goto Defend or if it's just Meredith that does it.
I have a theory that it happens like that because the enemy is trying to do something at the same time that Meredith does something, and yet i have succeeded in getting Meredith to make her move IF i waited until the enemy was finished..

Sounds like a battle engine bug beyond my control, though I've never heard of this particular error before. Try replacing Meredith with another character or moving her around in the party formation.
I've seen instances where during battles the turn gauges froze, but your description makes this bug seem systemic since it occurs every battle for the same round. Without the rm2k3 source code I can't really guess why this behavior is occurring. I guess I'll add this to my list of reasons to switch the game to the TURBU engine if it's ever finished. Sorry I couldn't help you out more.

Alright, I've started the game from scratch again and have discovered that this actually starts occurring in Nex Cacumen after you enter the cave. Maybe that could give you a lead on it

Maybe that could give you a lead on it Beomaust's Lair (the cave inside Nex Cacumen) does have a custom monster encounter system. It's possible this is causing the error, though it should not be since I've never been able to replicate this and no other player has reported this exact type of battle malfunction. There's nothing unusual in the battles themselves, and the custom encounter system really just keeps tracks of steps and calls a random battle manually when you've taken X amount of steps. For whatever reason this seems to be confusing the engine for you, but I don't think it's something I can fix. Probably something wrong in the rm2k3 source code. Thanks for following up though. I would imagine you'd get similar results entering the Incultus Regnum Basement since it uses the same encounter script.

I've just completed Incultus Regnum Basement right now and there doesn't seem to be much problem that I noticed here. (I've been fighting Hammer Ogre and Giant in IR Basement) - but I have seen this happen against Grand Cyclops though, and in Solus Insula against Magnate
Apparently it's not just limited to Beomaust's Lair, but right now I'm actually unsure if it's only special monster (the one where you get a beherit or the others) that's the problem.
I don't exactly remember now if it did or did not happen in some or all of the fights before I started the game from scratch, because I was out hunting in Beomaust's Lair after beherit's/X Source's then it may have seemed like it happened all the time.
Maybe the game just gets a glitch if any single character has the same speed as the monster they fight and then they try to pick a option at the same time.
Update : Allright, now I'm quite sure it's the monsters on some point that's doing this. I'd just been at the Battle Arena and discovered when I came to Nex Cacumen that they skipped one option in the first round.
